Enterohepatic, Gluco-metabolic, and Gut Microbial Characterization of Individuals With Bile Acid Malabsorption

Background and AimsBile acid malabsorption (BAM) is a debilitating disease characterized by loose stools high stool frequency. The pathophysiology of BAM not well-understood. We investigated postprandial enterohepatic gluco-metabolic physiology, as well gut microbiome composition fecal bile content in patients with BAM.MethodsTwelve participants selenium-75 homocholic taurine test–verified 12 healthy controls, individually matched on sex, age, body mass index, were included. Each participant underwent 2 mixed meal tests (with without administration the sequestrant colesevelam) blood sampling evaluation gallbladder motility; microbiota evaluated specimens.ResultsPatients increased synthesis assessed circulating 7-alpha-hydroxy-4-cholesten-3-one, content, concentrations glucose, insulin, C-peptide, glucagon. McAuley index insulin sensitivity was lower than that controls. In BAM, colesevelam co-administered reduced acids fibroblast growth factor 19 7-alpha-hydroxy-4-cholesten-3-one but did affect glucagon-like peptide 1 responses or other parameters. Patients low relative abundance bifidobacteria Blautia, Streptococcus, Ruminococcus gnavus, Akkermansia muciniphila.ConclusionPatients are an overproduction acids, greater profile indicative dysmetabolic prediabetic-like state, changes their potentially linking phenotype.
